Director of Business Development (SNF/LTC) — DFW Texas Expansion

Compensation: Competitive base uncapped bonus | OTE $100k–$125k
Paragon Post Acute Care is expanding rapidly across Dallas/Fort Worth and Texas growth markets. We’re hiring a proven SNF/LTC business development leader who knows PDPM, understands facility workflows, and can convert relationships into long-term facility partnerships.
Direct SNF/LTC/post-acute business development experience is REQUIRED.
This is a field-based, relationship-heavy role with high autonomy and high upside.
What You’ll Do
- Own Paragon’s facility growth pipeline across DFW and targeted Texas expansion counties
- Build relationships with SNF/LTC administrators, DONs, case managers, and physician partners
- Execute the full sales cycle: prospect → presentation → proposal → negotiation → onboarding
- Deliver 6–8 onsite visits weekly and consistent executive-level presentations
- Maintain accurate CRM tracking of leads, touchpoints, contracts, and launch timelines
- Collaborate with clinical and operations teams to ensure smooth go-lives
- Provide weekly, monthly, and quarterly reporting on activity and results
- Stay current on PDPM, Medicare/Medicaid, and managed-care workflows
Key Performance Expectations
- New facility contracts and onboardings per month/quarter
- New qualified leads added monthly
- Lead-to-contract conversion rate
- Weekly onsite visits
- Presentations delivered
- Revenue growth from new partnerships
- Facility satisfaction and retention
Required Qualifications
- 3–5 years healthcare business development / sales
- Direct SNF/LTC/post-acute experience (required)
- Strong understanding of SNF workflows and PDPM
- Proven track record of closing facility partnerships
- Comfortable with a lower-base, high-upside performance bonus structure
- Valid driver’s license and ability to travel locally/regionally
Preferred (Not Required)
- Bachelor’s degree
- Clinical background (RN/LVN)
- Existing Texas SNF/LTC decision-maker network
Territory & Travel
- Primary territory: Dallas/Fort Worth metroplex
- Expansion support: Texas statewide growth markets
- Field-based role with regular regional travel for presentations, relationship development, and go-lives
Compensation
- Competitive base salary uncapped performance bonus (“eat what you kill”)
- OTE $100k–$125k at target performance, with meaningful upside for over-performance
- Base typically $55k–$65k depending on experience
- Bonus tied directly to new facility wins, successful go-lives, and service-line expansion
