Job responsibilities include:
- Collaborating with physicians and multidisciplinary team to provide the best possible care.
- Identifies resident care requirements by establishing personal rapport.
- Delivers care in accordance with resident care plan, policies and procedures of PVE, and principles of relationship-based care.
- Assures quality of care by adhering to standards; measuring health outcomes against care goals and regulatory standards.
- Maintains safe and clean working environment by complying with regulations.
- Documents resident care services by charting.
- Maintains continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs.
- Maintains professional and technical knowledge by attending educational workshops (provided on-site).
- Other duties as assigned.
- Graduate from a credentialed nursing program
- Valid RN or LVN license in California
- Excellent communication skills (verbal and written)
- Ability to work independently and as part of a team
- Ability to adapt to unpredictable situations
- Must have flexibility to work rotating days, some weekends and some holidays
