The Nurse Practitioner is responsible for evaluating, diagnosing, and treating acute, episodic, and chronic health conditions as part of an interdisciplinary healthcare team. This role focuses heavily on wound management, chronic disease support, patient education, and continuity of care within an in-office clinical environment.
This is an in-office position requiring the provider to work from Company clinical locations and centralized systems. Responsibilities include management of in-office patient care, telehealth encounters, remote therapeutic monitoring (RTM) activities, chart review, care coordination, and clinical oversight functions. Remote work from home is not contemplated within this role.
The Nurse Practitioner will support centralized clinical operations and may provide in-person patient visits as operational needs and schedule availability allow. The role requires professionalism, adaptability, strong clinical judgment, and the ability to function efficiently in a fast-paced healthcare environment.
Essential Duties and ResponsibilitiesThe essential functions include, but are not limited to, the following:
- Conduct and document telehealth encounters in accordance with Company policy and applicable regulations
- Review and manage remote therapeutic monitoring (RTM) data, wound images, and patient-reported clinical information through Company-approved platforms
- Monitor and respond appropriately to changes in patient condition identified through centralized monitoring systems
- Coordinate follow-up care, treatment recommendations, and escalation of care when clinically indicated
- Support centralized clinical workflows and patient management initiatives
- Provide in-person patient visits as operational needs and schedule availability permit
- Maintain proficiency with telehealth, RTM, electronic medical record systems, and other healthcare technology platforms
- Perform comprehensive patient evaluations and ongoing reassessments
- Recommend and order diagnostic or therapeutic interventions with consideration to safety, efficacy, patient adherence, and cost-effectiveness
- Prescribe medications and treatments within scope of licensure and applicable regulations
- Educate patients and caregivers regarding disease processes, wound care, medications, prevention strategies, and self-management techniques
- Collaborate with physicians, nurses, therapists, office staff, and other healthcare professionals to support coordinated patient care
- Review, complete, and maintain accurate clinical documentation within Company-defined timelines
- Participate in chart review, intake review, telehealth encounters, remote monitoring activities, care coordination, and treatment planning activities
- Utilize electronic medical records and other healthcare technology systems efficiently and accurately
- Maintain professionalism and adaptability in a dynamic clinical environment
- Demonstrate willingness to learn and utilize advanced wound care products, medical equipment, and evolving treatment modalities
- Support operational workflows and clinic efficiency initiatives as assigned
- Perform other clinical, administrative, or operational duties consistent with the role and scope of practice to support patient care and Company operations
- Active and unrestricted Nurse Practitioner license
- Preferred prescriptive authority
- Master’s degree in Nursing or higher
- Preferred minimum of five (5) years of nursing experience
- Preferred wound care experience
- Strong clinical judgment and critical thinking skills
