Role
We are seeking a full-time Intake Coordinator to join our growing team. This role is perfect for someone who thrives on organization, multitasking, and meaningful interactions with patients, families, and referral partners. As the first point of contact for new patients, you’ll play a vital role in ensuring a seamless experience:
What You’ll Do:
- Receive and document incoming referral calls.
- Review referrals to ensure completeness and accuracy.
- Enter patient information into our management system.
- Act as a key communication link between referral source, patients, and sales members.
- Handle additional administrative duties as needed.
- Licensed Vocational Nurse
- Experience: 12 years working with intake and scheduling in a licensed home health agency.
- Technical Skills: Proficient in advanced office computer applications.
- Soft Skills: Excellent customer service, communication, organizational, and interpersonal abilities.
- Professionalism: Able to manage multiple priorities while maintaining a positive, teamfocused attitude.
