Job Description
- Full-time Hospice RN On-Call
- Friday 5:00pm-11:00 pm; Saturday/Sunday 8:00 am - 11:00 pm
- Territory: We respond to On-call needs within an hour
- Performs physical and psychosocial assessment on all hospice patients
- Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management
- Responds to the physical, mental, social and emotional needs of terminally ill patients and their families
- Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary
- Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate
- Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc

- One year Hospice experience required
- 1 year of RN experience required
- Licensed Registered Nurse in State of Indiana
- Current driver's license
- Must have reliable transportation and insurance
