What You’ll Do
- Complete skilled nursing assessments and develop individualized care plans.
- Provide hands‑on care including wound care, catheter/ostomy care, medication management, and more.
- Monitor patient progress and adjust care plans as needed.
- Educate patients and families on disease management, medications, and self‑care.
- Document all care accurately in the electronic medical record.
- Collaborate with a multidisciplinary team to ensure seamless, high‑quality care.
What's Required
- Valid RN license in West Virginia.
- Current CPR certification (Basic Life Support).
- Nursing experience (acute care or home health preferred).
- Strong assessment skills and sound clinical judgment.
- Excellent communication and interpersonal skills.
- Comfort working with diverse patient populations.
