The Advanced Practice Nurse is responsible for providing direct client care across preventative, acute, and chronic health conditions and diseases. This includes assessing, diagnosing, planning, and prescribing pharmacologic and non-pharmacologic treatments, therapeutic and corrective measures, and health promotion and preventive care. Core clinical services include reproductive health for women and men, immunizations for children and adults, and primary care. The scope of care may expand based on identified needs within the community and per the department.
Qualifications and Proficiencies:
- Provide direct patient care, including medical evaluation, diagnosis, treatment, and prescription management.
- Perform and interpret laboratory screenings, prescribe medications per established protocols, and follow up on abnormal results and health conditions.
- Follow up on abnormal test results and manage health conditions for clients across their lifespan.
- Administer immunizations for children and select vaccines for adults, addressing vaccine misconceptions and providing accurate information to promote recommended vaccinations as needed.
- Assess and manage clients with complex medical symptoms, challenging diagnoses, and the presence of disease, while addressing social, emotional, or behavioral factors that may impact treatment adherence, including substance abuse or history of abuse within the scope of practice.
- Collaborate in activities that contribute to the development of budgets and the efficient use of resources.
- Meet regularly with the Lead Nurse Practitioner and/or Clinic Manager to review the financial performance of clinic services and patient visit performance standards.
- Make practical use of the local Health Community Assessment and other local health status data to determine the proper focus of health promotion efforts.
- Provide oversite to MA’s and nursing staff in clinics in which they service
- Complete all required screenings as required by grant funding sources
- Room patients as required due to staff outages or low census
- Participate in flu clinic events as assigned
- Other duties as assigned and consistent with job classification
Physical Requirements:
Due to the need for all Health Department personnel to respond to public health emergencies, the employee must be assessed for his/her ability to meet the physical demands of performing the following activities:
- Engage in the following movements: climbing, balancing, stooping, kneeling, crouching, reaching, standing, walking, lifting, fingering, grasping, feeling, and writing and repetitive motions.
- To move objects, exert up to 30 pounds of force occasionally and/or a negligible amount of force constantly.
- Hearing ability sufficient to communicate effectively in person and over the phone.
- Visual ability must be sufficient to read typewritten documents, computer screens, and drive a car
