What are the responsibilities and job description for the Diabetes Educator position at Pancare of Florida Inc?

Your responsibilities include the following:
- Conducting Diabetes Self-Management Education and Training (DSME/T) participant assessments using the AADE7 framework
- Developing measurable goals, teaching plans, and individualized learning objectives
- Providing education using interactive and skill-based methods to increase engagement and understanding
- Documenting patient assessments, learning plans, progress, and evaluations outcomes
- Performing follow-up assessments and track participant progress toward defined goals
- Referring patients to appropriate health providers as needed
- Communicating relevant findings and updates to the primary care provider and healthcare team
- Participating in developing program and teaching materials
- Supporting program accreditation requirements and participate in accreditation surveys
- Engaging in continuous quality improvement initiatives for Diabetes Self-Management Education and Support (DSMES)
- Maintaining required annual continuing education specific to diabetes and behavioral change strategies
- Staying current with advancements in diabetes management, technology and educational best practices
Required Qualifications:
- Active RN or LPN License
- At least 1 year experience with DSME/T programs
- At least 1 year experience developing therapeutic relationships and motivating participants
- Strong understanding of diabetes treatment and self-management care
- Ability to present education content
- Strong communication and teaching skills
Preferred Qualifications:
- Certified Diabetes Educator (CDE) or experience working in diabetes education preferred
- At least 3 years experience with DSME/T programs
- At least 3 years experience developing therapeutic relationships and motivating participants
