Responsibilities
- Perform comprehensive patient assessments and develop individualized care plans in collaboration with the care team.
- Complete evaluation tasks, including reviewing medication and vital signs.
- Coordinate care by making appropriate referrals or documenting clinical rationale when referrals are not indicated.
- Communicate effectively with the clinical manager and interdisciplinary team regarding patient status and care updates.
- Educate patients and caregivers to promote independence and improve health outcomes, including training in self-care techniques.
- Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
- Supervise and support LPNs to ensure high standards of care.
- Perform additional duties as needed to support patients and team operations.
Qualifications
- Current Registered Nurse (RN) license in the state of Florida
- Must have at least 1 year experience as a home health nurse
- Excellent knowledge of nursing care methods and procedures
- In-depth knowledge of health and safety guidelines and procedures
- A team player with excellent communication and interpersonal skills
- Outstanding organizational and multi-tasking skills
- Patient with excellent problem-solving skills
- Familiar with EHR such as Kinnser.
- Ability to work independently, manage a daily schedule, and travel between patient homes
