Position Summary
The Intake Coordinator is responsible for receiving and processing home health referrals while coordinating patient admissions and scheduling with clinicians. This role serves as the primary point of contact for referral sources and works closely with physicians, hospitals, and the clinical team to ensure timely, accurate, and exceptional patient care.
Responsibilities
Receive, review, and process home health referrals.
Enter patient and referral information accurately into the EMR.
Coordinate Starts of Care (SOC), Resumptions of Care (ROC), and patient scheduling.
Assign clinicians based on coverage, availability, and patient needs.
Track pending referrals and follow up to ensure timely admissions.
Coordinate admissions for patients discharging from hospitals or facilities.
Communicate with physicians, referral sources, patients, and clinical staff.
Verify insurance and obtain authorizations when applicable.
Maintain accurate documentation and ensure HIPAA compliance.
Assist with additional intake and scheduling duties as assigned.
Qualifications
Minimum 1 year of home health intake, scheduling, or healthcare office experience preferred.
Knowledge of medical terminology.
Experience with insurance verification is a plus.
Experience with WellSky or other EMR systems preferred.
Strong organizational, communication, and customer service skills.
Ability to multitask and work in a fast-paced environment.
Proficiency in Microsoft Office.
