What are the responsibilities and job description for the Field Nurse Case Manager position at Palmetto Case Management?
Palmetto Case Management is looking for a field nurse case manager with a rehabilitation and/or emergency trauma background. Workers Compensation experience is a plus. Palmetto nurse case managers follow the injured person's care and treatment plans from start to finish, facilitate medically necessary services and address long-term needs, coordinate the different levels of care necessary, bridge communication between all parties, including the injured person, family, medical providers, insurance carriers and the employer and provide ongoing support to the injured person and their family.
Home office plus travel between worksites and clients. Position requires driving of personal vehicle for completion of tasks.
