Targeted Case Manager (TCM) — Paintsville Behavioral Health TRP

About Us: Paintsville Behavioral Health is a Therapeutic Rehabilitation Program (TRP) dedicated to improving the quality of life for adults experiencing serious mental illness and co-occurring disorders. Position Summary: The Targeted Case Manager (TCM) is responsible for providing individualized case management services to TRP participants. This role serves as a vital link between clients, providers, and community supports—ensuring that each participant’s needs are assessed, coordinated, and addressed through a holistic, strengths-based approach. The TCM will also serve as a liaison between the TRP and local Personal Care Homes to promote continuity of care, communication, and advocacy for client needs.
Key Responsibilities:
- Conduct comprehensive assessments and develop individualized service plans.
- Coordinate and monitor behavioral health, medical, housing, and community services.
- Maintain regular contact with clients to assess progress toward recovery goals. Local travel will be required.
- Serve as liaison between TRP staff, personal care homes, and outside providers.
- Document services and maintain compliance with Medicaid and DBHDID requirements.
- Participate in multidisciplinary team meetings and case reviews.
- Support client advocacy, empowerment, and community reintegration efforts.
Qualifications:
- Bachelor’s degree in a human services-related field (psychology, social work, counseling, etc.) — required.
- Current Targeted Case Management (TCM) certification through the Kentucky Department for Behavioral Health, Developmental and Intellectual Disabilities (DBHDID) — required.
- Prior experience in behavioral health, social services, or case management preferred.
- Strong communication, organization, and documentation skills.
- Ability to work collaboratively as part of a multidisciplinary team.
