Position Summary
We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) or Medical Assistant (MA) to provide Principal Care Management (PCM) services in either a fully remote or onsite clinic setting. This role focuses on supporting patients with a single high-risk chronic condition through consistent, proactive engagement and care coordination.
Working under the supervision of a physician or qualified healthcare provider, you will play a key role in improving patient outcomes by monitoring health status, reinforcing care plans, and ensuring continuity of care. Key Responsibilities
- Enroll eligible patients into the Principal Care Management (PCM) program
- Maintain ongoing patient engagement and participation in PCM services
- Conduct regular patient outreach via phone and secure messaging
- Serve as a consistent point of contact for assigned patients
- Assess patient-reported symptoms and medication adherence
- Identify and escalate significant changes in patient condition to the care team
- Reinforce and support implementation of individualized care plans
- Provide patient education to promote self-management and treatment adherence
- Coordinate care services, including specialist referrals, diagnostic testing, and follow-ups
- Accurately document all patient interactions in the electronic medical record (EMR)
- Track and report time spent on PCM activities in compliance with program requirements
Qualifications
- Active, unrestricted LPN license or Medical Assistant certification in Georgia or a valid compact license
- Prior experience in care coordination, chronic care management, or remote patient care preferred
- Proficiency with Electronic Medical Records (EMR) systems and Microsoft Office
- Strong verbal communication skills, especially via phone-based patient interactions
- Excellent organizational skills and attention to detail
- Ability to work independently while maintaining consistent productivity and documentation standards
