Position Summary: The Case Manager will provide the assessment and the development of the ongoing behavioral plan which will structure the individualized program for each resident. He/she will be the primary liaison between the resident, treatment providers, and his school, family and community.
Qualifications:
- Must have bachelor’s degree preferred in social work, behavior or related discipline.
- Has not been convicted of crimes against persons or drug related crimes as evidenced by a criminal background check.
- Must be competent in reading and writing English.
- Certified in CPR and First Aid.
- Must be at least 21 years of age.
- Valid driver’s license and current automobile insurance.
- Must be able to work flexible hours.
- Experience working in residential setting a plus.
Primary Responsibilities:
- Monitor weekly behavior and improvements within social interaction.
- Provide referral sources with weekly updates on child’s progress in treatment Monitor and support the social, emotional, behavioral, and academic needs of students.
- Email/phone call to referral source at least once per week.
- Email/phone call to family/guardian at least once per week.
- All contact should be documented in case management notes.
- Case management notes should be printed weekly.
- Document time and resources in child’s treatment process.
- Weekly documentation of meetings with the child.
- Weekly documentation of progress of treatment plan goals.
- Construct monthly reports for every 30 day reporting period.
- Send to Dr. Parker to review.
- Send report to referral source.
- Attach fax confirmation sheet and place in to be filed.
- Monitor Court Dates.
- Case managers will be responsible for documenting upcoming court dates for each resident on their caseload.
- Attend court hearings for residents.
- Other duties as assigned.
- Travel to meetings and conferences as needed.
- Additional duties as assigned by the supervisor.
- On-Call duties as outlined in the on-call duties description.
- Work as a team member within PVRC to insure effective case management for all residents in the program.
Required Skills, Knowledge and Abilities
- Knowledge of: principles and practices of effective clinical case; the principles and practices of effective program management; expanded principles of psychology, social work, juvenile justice and other social sciences; family systems approach to treatment; child and adolescent development (needs and behaviors); crisis intervention and management; needs assessment techniques and strategies; problem-solving and conflict resolution; symptoms and effects of physical, emotional, sexual abuse and neglect; substance abuse and its indicators; parenting skills and behavior management techniques; community resources and services; cultural diversity and value systems; mental health, juvenile justice, education and related systems services.
- Skill in: coordinating resident’s care, evaluating services/systems of care, developing individual plans of care, efficient and effective written and oral communication; and interfacing effectively with stakeholders.
- Ability to: relate with children through understanding and respect; evaluate and display insight into individual behavior; use supervisory skills to enable professional and paraprofessional staff development; follow all safety and precautionary rules and measure; prioritize and self-motivate to accomplish job goals and perform work efficiently and effectively under stressful situations; use available resources; and demonstrate effective discharge planning and follow-up.
- Physical demands: Must be able to lift, sit, stand, bend, or stoop for long periods of time. Possess the sensory capacities to efficiently and successfully execute essential job functions.
