A valid Speech and Language Pathology Assistant license issued by the Speech-Language Pathology and Audiology Board (SLPAB).

Minimum of 60 College Credits Completed

Certification (A valid Speech and Language Pathology Assistant license issued by the Speech-Language Pathology and Audiology Board (SLPAB).)
Copy of Transcript (must be an Official Transcript)
Letter of Introduction
Letter(s) of Recommendation (2 within the last 12 months)
Resume
