Position Overview

The Advanced Practice Provider will manage the ongoing medical needs of residents in skilled nursing and long-term care facilities in the Helena area.

You will have the opportunity to develop long-term relationships with residents and facility staff while managing both acute concerns and chronic medical conditions.


This position is ideal for a provider who:

Key Responsibilities

Patient Care

  • Perform comprehensive history and physical examinations
  • Evaluate, diagnose, and manage acute and chronic conditions
  • Develop individualized treatment and care plans
  • Order and interpret laboratory testing, imaging, and other diagnostics
  • Prescribe medications and therapies within scope of practice
  • Provide post-hospital and transitional care follow-up
  • Address cognitive, behavioral, functional, and medical changes
  • Educate residents and families regarding diagnoses and treatment plans

Skilled Nursing and Long-Term Care

  • Complete admission assessments and required documentation
  • Conduct routine and medically necessary follow-up visits
  • Participate in interdisciplinary care planning
  • Collaborate with facility nursing, therapy, dietary, social services, and administrative teams
  • Support initiatives that reduce avoidable emergency department visits and hospital readmissions
  • Maintain accurate and timely EMR documentation
  • Review and sign laboratory results promptly
  • Follow applicable CMS, state, and facility requirements

Collaboration

  • Work closely with an assigned collaborating physician
  • Communicate effectively with facility staff, families, caregivers, and specialists
  • Participate in multidisciplinary patient management
  • Help create a consistent, responsive, and patient-centered clinical presence within the facility


Qualifications

Required

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program
  • Active and unrestricted Montana license, or ability to obtain one
  • National board certification or board eligibility by the start date
  • DEA registration and prescriptive authority, or ability to obtain them
  • NPI number
  • Eligibility to participate in Medicare and Medicaid
  • Current BLS certification
  • U.S. work authorization

Preferred

  • Experience in skilled nursing, long-term care, geriatrics, internal medicine, family medicine, or post-acute care
  • Experience managing medically complex patients and chronic conditions
  • Familiarity with CMS documentation and regulatory requirements
