RN Case Manager$5,000 Sign-On BonusPosition SummaryThe RN Case Manager is responsible for coordinating and overseeing the delivery of rehabilitation and medical services for residents in a Skilled Nursing Facility. This role collaborates closely with the interdisciplinary team to ensure cost-effective, high-quality care while facilitating admissions, ongoing utilization review, and discharge planning.
