Job Summary
The CFT Specialist is responsible for facilitating initial and ongoing Child and Family Team Meetings for families involved with Sacramento County CPS (Court Services and Permanency Services). Specifically, the CFT Specialist is responsible for facilitating time limited, outcome-focused Child and Family Team (CFT) Meetings with families, extended family, children/youth, caregivers, community representatives, service providers, and agency staff for all case planning and placement related decisions in partnership with Child Protective Services (CPS). The CFT Specialist guides meeting participants through discussion of concerns, identification of strengths, and development of ideas to reach consensus regarding the least restrictive, best possible case planning and placement-related decisions. The CFT Specialist is required to complete session notes. The CFT Specialist works in a primarily office-based setting, under the guidance of a Clinical Program Manager. The CFT Specialist works within the vision, mission and philosophy of the agency and the Department of Child, Family and Adult Services.
Essential Duties
- Acts as a neutral facilitator of pre-scheduled CFT meetings and provides support to the child and family team to problem solve and make decisions regarding placement, and/or stabilization. Provides strength-based family centered facilitation methodology during CFT meetings and supports the CFT to develop creative methods to ensure individualized and culturally sensitive care. CFT team meetings will be arranged by department scheduler.
- Facilitates the Child and Family team in the development of their own Action Plans that include functional strengths and needs of the child/youth and family based on priority needs across life domains to develop a child/youth and family-centered action plan. Written by the county social worker, the action plan articulates specific strategies for achieving the child/youth and/or family’s goals based on functional strengths, identified needs, public safety, and any related court orders.
- Understands interactions of Child Protective Services, Probation, Mental Health Providers, Educational System and other complex systems in the context of a family’s life, service delivery structures and the community’s resources.
- Understands and elicits the child and family values and incorporates divergent perspectives in the plans and strategies in order to help youth stabilize in their placement.
- Facilitates the distribution of written copies of the Action Plans and Attendance Sheets to all Child and Family Team members.
- Demonstrates effective personal organizational systems for ensuring paperwork and documentation is entered with accuracy and timeliness.
Other Duties and Responsibilities
- May be called upon to drive to other agency or site locations.
- May be called upon to drive to alternate locations in the community in order to conduct a CFT.
- Physical interactions with children of variable ages such as but not limited to games, sports, and other activities.
- Performs other related responsibilities as assigned to support specific department/business needs.
Education Requirements
- Bachelor’s Degree (BA/BS) required
PREFERRED EDUCATION
- Bachelor’s Degree (BA/BS) in Social Work, Behavioral Science, Education or related field
Experience Requirements
- One (1) year experience working with youth/families with complex needs. Experience with social work, child welfare, juvenile probation systems, and/or mental health systems required
PREFERRED EXPERIENCE
- One (1) year experience Social Work, Healthcare/Medical – Psychology/Behavioral/Mental Health
HIGHLY DESIRABLE QUALIFICATIONS
- Speak, read, and/or write another language fluently
- Experience facilitating Child and Family Team Meetings
- Experience and Knowledge of Safety Organized Practice and Sacramento County Child Protective Services
Other Specific Requirements
- Must pass Department of Justice (DOJ), Federal Bureau of Investigations (FBI), and Child Abuse Index Check (CAIC) background clearance.
- Valid California Driver's license with two years of experience driving and clean driving record may be required.
