POSITION OVERVIEW
The RN Case Manager will provide telephonic care management and coordination for Dual Eligible Special Needs Plan (D-SNP) members, focusing on Transition of Care (TOC), clinical assessments, individualized care planning, member engagement, and closure of HEDIS/Medicare STAR quality gaps. The role requires a Registered Nurse with strong clinical case management skills, experience coordinating post-discharge care, and knowledge of Medicare Advantage and D-SNP requirements.
ROLES & RESPONSIBILITIES
- Conduct telephonic outreach to members and caregivers following hospital, SNF, rehabilitation, or emergency department discharge.
- Perform clinical, psychosocial, functional, and Social Determinants of Health (SDOH) assessments.
- Develop and update individualized care plans based on member needs and health risks.
- Coordinate care with providers, multidisciplinary teams, caregivers, and community organizations.
- Support Health Risk Assessments (HRAs), Individualized Care Plans (ICPs), and Interdisciplinary Care Team (ICT) activities..
