Job Title: Spanish-Speaking Outreach Specialist

Position Summary:
The Outreach Specialist plays a vital role in the growth and visibility of the PACE (Program of All-Inclusive Care for the Elderly) program within Spanish-speaking communities. This community-facing role is responsible for building relationships, conducting outreach and education, and supporting enrollment efforts for eligible older adults. The ideal candidate is a fluent Spanish speaker with strong interpersonal skills, cultural competence, and a passion for helping underserved populations access high-quality, holistic care.
Key Responsibilities:
- Serve as a trusted ambassador of the PACE program within Spanish-speaking communities.
- Identify and attend community events, senior centers, churches, and other venues to promote awareness of PACE services.
- Educate individuals, families, and community partners about the benefits of the PACE model and the eligibility/enrollment process.
- Build and maintain relationships with community organizations, healthcare providers, and other stakeholders.
- Distribute outreach materials and ensure they are culturally and linguistically appropriate.
- Conduct home visits or site visits, as needed, to assist potential participants and their families.
- Collaborate with the PACE enrollment team to support referrals and facilitate the intake process.
- Maintain accurate records of outreach activities and outcomes; provide regular reports to leadership.
- Provide feedback to improve outreach strategies and materials.
- Ensure compliance with all HIPAA and confidentiality requirements.
Qualifications:
- Fluency in Spanish and English (spoken and written) required.
- Strong understanding of and sensitivity to the needs of Spanish-speaking and multicultural communities.
- Excellent verbal communication and public speaking skills.
- Prior experience in community outreach, health education, social services, or related fields preferred.
- Ability to work independently and as part of a team.
- Valid driver’s license and reliable transportation; willing to travel locally for outreach activities.
- Proficiency in Microsoft Office (Word, Excel, PowerPoint) and basic data entry.
- Knowledge of PACE or senior care services is a plus, but not required.
