Field Nurse: Advocacy & Case Management
Are you ready to trade the clinical floor for the open road? We are seeking an experienced LPN, HHA, CHW to provide critical advocacy and health service coordination across a five-county territory. This role is designed for the independent professional who thrives on variety and self-management.

The Elks Home Service Program provides information, referral and advocacy services to families with children with special healthcare needs, and adults with developmental disabilities that live in the community. Help parents navigate the complicated systems that their children are involved with, and support parents as they advocate for what their children need to succeed. This position covers Bucks, Lehigh, Montgomery, Northampton and Philadelphia Counties, and a company car is provided for work travel.
