What are the responsibilities and job description for the Direct Service Provider position at PA Child?
GENERAL JOB DESCRIPTION: Direct Support Providers deliver services to individuals with developmental and intellectual disabilities both in the home and community.
DUTIES & RESPONSIBILITIES:
· Delivering services in accordance with each individual’s individual support plan and behavior plan
· Establish a relationship based on understanding and compassion while remaining professional and respecting ethical boundaries/guidelines.
· Support individuals in achieving the highest level of independence in areas targeted by goals
· Assist individuals with budgeting, financial transactions and planning
· Helping individuals in navigating and understanding social interactions/situations across various settings
· Assisting individuals with achieving community integration and making meaningful connections within their communities
· Transport individuals to essential locations in the community including but not limited to: medical appointments, goal focused and/or leisure activities
· Properly document all encounters and gather data, in a timely fashion, that will contribute to individuals monthly/quarterly evaluations of progress.
· Attend and participate in monthly/quarterly/annual meetings and evaluations
· Advocate for individuals’ wants and needs while assisting them to advocate for themselves
· Communicate effectively to other team members including family, caregivers, friends and other professionals in a clear and professional manner
· Complete all pre-service training and annual training as required
· Any other duties assigned by director
QUALIFICATIONS FOR THE JOB: The applicant must be at least 18 years of age, have a high school diploma or GED, and a valid PA drivers license. Applicants must also have reliable transportation with current registration, insurance, and inspection.Must complete PA State Police Clearance and driving history.

Pay: $18.00 - $20.00 per hour
Expected hours: 18.0 – 20.0 per week
