Working with P3 health Partners
By joining our team, you and your patients have access to our local team of experienced professionals, which includes MDs, NPs, and RNs.
*This position can be part-time or full time hours.
Responsibilities:
- Conduct comprehensive Annual Wellness Visits in compliance with CMS guidelines.
- Perform health risk assessments and screenings.
- Review current medications, reconcile lists, and assess adherence.
- Identify risk factors, gaps in care, and potential health concerns.
- Provide patient education on preventive health measures and chronic condition management.
- Document all encounters accurately and timely in the EMR system.
- Utilize tools to manage clinical and financial resources.
Qualifications:
- Current, unrestricted Nurse Practitioner license in California
- Training or interest in geriatrics welcome, but not required.
- Current DEA license.
- Board Eligible or Board Certified in Family or Internal Medicine.
- Excellent communication, documentation, and interpersonal skills.
