Position Summary
We are seeking a compassionate and detail-oriented Registered Nurse (RN) to provide clinical oversight for our non-skilled Home and Community Based Services (HCBS) program in Missouri. This part-time position plays a critical role in ensuring participant safety, regulatory compliance, and quality personal care services for elderly and disabled clients living in the community.
The RN works closely with personal care aides, participants, and the Division of Senior and Disability Services (DSDS) to monitor care plans, conduct nurse visits, and support staff competency and training.
Duties and Responsibilities
- Conduct initial and periodic in-home assessments to evaluate participant health status and adequacy of the Person-Centered Care Plan.
- Monitor participant conditions and identify significant physical or mental health changes.
- Report condition changes to DSDS and request care plan revisions when necessary.
- Perform maintenance and preventative nurse visits for participants requiring enhanced supervision beyond standard personal care services.
- Conduct required monthly on-site supervisory visits for participants receiving APC services.
- Evaluate aide performance and participant physical condition during APC visits.
- Ensure compliance with Medicaid and state HCBS regulations.
- Organize and set up oral medications in pill planners for participants who cannot independently manage medications but are able to self-administer once prepared.

Observe and document personal care aide competency in tasks such as:
- Bathing and grooming
- Infection control
- Specialized APC tasks including ostomy care and catheter hygiene
- Provide immediate on-the-job coaching and instruction when safety or protocol concerns are identified.
- Conduct routine performance evaluations of direct care staff.
- Audit Electronic Visit Verification (EVV) records and maintain accurate documentation.
- Ensure timely completion of required supervisory visits and care plan reviews.
Job Conditions
- Field-based position requiring travel to participant homes throughout assigned service areas
- Hybrid schedule with both field visits and office/administrative work
- Exposure to varying home environments and medically complex participants
- Requires strong clinical judgment, communication, and organizational skills
- Reliable transportation required
Qualifications
Applicants must:
- Be at least 21 years of age
- Currently licensed Registered Nurse in Missouri
- Successfully pass a criminal background check
