Job Summary

Responsibilities

  • Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner.
  • Administer medications and injections accurately following physician orders while ensuring compliance with HIPAA and other confidentiality policies.
  • Manage IV infusions, catheter care, tube feeding, and ventilator support for patients requiring specialized nursing interventions.
  • Conduct diagnostic evaluations such as specimen collection, processing laboratory tests, and utilizing medical imaging when necessary.
  • Provide wound care, including suturing and aseptic techniques, to promote healing and prevent infections.
  • Assist with complex procedures like cardiac catheterization or spinal taps in collaboration with physicians and specialists.
  • Coordinate discharge planning, case management activities, and patient education to ensure seamless transitions of care across settings.
  • Document all patient interactions thoroughly in EMR/EHR systems like Athenahealth or eClinicalWorks while maintaining clinical confidentiality policies.
  • Support patients with dementia or Alzheimer's disease through memory care strategies and behavioral health interventions tailored to geriatric populations.
  • Participate in emergency response situations such as acute pain management or airway management during urgent medical episodes.

Experience

  • Valid Licensed Practical Nurse (LPN) license with current state certification required.
  • Proven experience working with diverse patient populations across multiple healthcare settings including hospitals, skilled nursing facilities, hospice care, or home & community care environments.
  • Critical care experience preferred, especially within ICU or Level I Trauma Center settings involving trauma or infectious disease care.
  • Familiarity with ICD coding (ICD-9/10), CPT coding, medical documentation standards, and EMR/EHR systems such as Cerner or Epic is highly desirable.
  • Prior experience in long-term care facilities or assisted living communities demonstrating expertise in geriatrics and dementia care preferred.
  • Knowledge of physiology, anatomy, pharmacology related to medication administration and pain management is essential.
  • Ability to perform triage assessments in urgent care or emergency room settings efficiently and accurately.
  • Experience working with individuals with disabilities or developmental disabilities is a plus to support diverse patient needs effectively.
