Position Summary

Ozark Behavioral Health is seeking a compassionate, knowledgeable, and organized Registered Nurse (RN) to provide nursing oversight for consumers receiving services through our Individualized Supported Living (ISL) program.
The RN will be responsible for monitoring the health and well-being of assigned consumers, completing required monthly nursing assessments, coordinating medical care, providing clinical guidance to staff, and overseeing LPN services. This position works closely with behavioral health professionals, direct-care staff, physicians, families, and other healthcare providers to ensure consumers receive safe, appropriate, and quality care.
The ideal candidate will have strong clinical judgment, excellent communication skills, attention to detail, and a passion for supporting individuals with behavioral health and developmental needs in a community-based setting.

Essential Duties and Responsibilities

  • Provide nursing oversight and clinical support for consumers residing in Individualized Supported Living (ISL) settings.
  • Complete monthly nursing assessments and other required nursing assessments in accordance with organizational, regulatory, and program requirements.
  • Monitor consumers' overall health status and identify changes in condition requiring intervention or follow-up.
  • Review medical conditions, medications, treatment plans, physician orders, and other pertinent health information.
  • Coordinate and follow up on physician appointments, specialist visits, laboratory testing, hospitalizations, and other medical services as needed.
  • Communicate changes in a consumer's condition to appropriate healthcare providers, supervisors, family members, and other members of the care team.
  • Provide clinical guidance and support to ISL staff regarding consumers' healthcare needs.
  • Provide clinical oversight and supervision of LPN staff, including reviewing nursing responsibilities, documentation, assessments, and follow-up care.
  • Monitor LPN performance and provide direction, coaching, and feedback related to clinical responsibilities.
  • Review and ensure appropriate implementation of nursing interventions and physician orders.
  • Monitor medication-related concerns and collaborate with appropriate staff regarding medication management and changes.
  • Assist with development, implementation, and monitoring of individualized healthcare plans.
  • Ensure nursing documentation is accurate, timely, complete, and maintained in accordance with organizational policies and applicable regulations.
  • Monitor consumers for changes in physical or behavioral health and initiate appropriate interventions.
  • Provide education to consumers, families, caregivers, LPNs, and direct-care staff regarding health conditions, medications, treatments, and preventive healthcare.
  • Participate in interdisciplinary team meetings and contribute nursing recommendations to individualized service plans.
  • Follow up on emergency room visits, hospital admissions/discharges, injuries, illnesses, and other significant health events.
  • Assist with incident reviews and provide nursing recommendations when a consumer experiences an injury, medical emergency, or significant change in condition.
  • Maintain knowledge of applicable CMS, state, Medicaid, behavioral health, and organizational requirements.
  • Participate in quality assurance activities and assist with identifying opportunities to improve consumer health outcomes and service delivery.
  • Maintain confidentiality and protect consumer rights and personal health information in accordance with HIPAA and organizational policies.
  • Perform other duties as assigned by the Director of Behavioral Health.

Qualifications

  • Current, unrestricted Registered Nurse (RN) license in the state of Missouri.
  • Graduate of an accredited nursing program.
  • Previous nursing experience required.
  • Experience working with individuals with behavioral health, developmental disabilities, intellectual disabilities, or complex medical needs preferred.
  • Current CPR/BLS certification or ability to obtain certification as required.
  • Valid driver's license and reliable transportation as required for the position.
  • Ability to successfully complete all required background checks and agency requirements.

Physical Requirements

The employee must be able to perform the essential functions of the position. The position may require frequent standing, walking, bending, reaching, sitting, and movement between ISL locations. The RN may occasionally need to assist with or coordinate consumer care involving mobility limitations or other physical needs.
