Case Manager
Overview
Join our team and support clients through meaningful, person centered case management. This role focuses on guiding individuals through treatment, coordinating care, and ensuring high quality service delivery while working alongside a compassionate and collaborative team.
Responsibilities
Completes case management assessments, relapse prevention plans, and maintains consents.
Develops and monitors client care plans.
Builds linkages and refers clients for additional support services and between treatment modalities.
Refers clients in need of mental health and/or substance use services to appropriate community resources.
Documents all services provided to assure regulatory compliance with:
a. South Carolina Department of Alcohol and Other Drug Abuse Services (DAODAS)
b. SAMSHA
c. Federal Confidentiality Regulations contained in 42 Code of Federal Regulations Part 2
d. Federal HIPPA RegulationsWorks cooperatively and collaboratively with the entire Owl’s Nest team.
Coordinates and facilitates psychoeducation, social, and personal development groups for clients.
Maintains current case records and required documentation, preparing all necessary reports on time.
Communicates with appropriate referral sources, family, and employers regarding client care and counseling, in collaboration with the treatment team.
Provides case management duties in line with established standards and policies, preparing aftercare plans with the treatment team.
Ensures documentation and delivery of treatment services and monitors client progress as defined in the Owl’s Nest policy and procedures manual.
