Job Summary
Overstanding Wellness is seeking dedicated and compassionate QMHP-CS/Mental Health Case Managers to join our growing team. As a Case Manager, you will play a vital role in delivering high-quality mental health services to clients through telehealth and in-person appointments. You will work closely with clients to implement individualized treatment plans, ensuring they receive the support and resources necessary for their well-being and recovery.
Responsibilities
- Provide mental health services to clients based on their individualized treatment plans.
- Conduct assessments and develop treatment plans tailored to each client's needs.
- Deliver services both via telehealth and face-to-face, as required.
- Collaborate with clients, families, and other care team members to address mental health goals.
- Maintain accurate and up-to-date documentation in compliance with HHSC and TRR guidelines.
- Monitor and evaluate client progress, making adjustments to treatment plans as necessary.
Requirements
- Bachelor's or Master's degree in a mental health-related field (e.g., Psychology, Social Work, Counseling).
- Certification in CANS and ANSA.
- Completion of HHSC-approved training.
- Ability to deliver services via telehealth and in-person.
- Strong organizational and communication skills.
- A commitment to providing compassionate and client-centered care.
