Outreach is hiring a Young Adults Case Manager at Outreach Carol Stream to lead case management services for young adults in crisis or seeking to improve their circumstances, supporting the mission of Outreach to restore hope and provide opportunities for individuals to become all that God intends them to be.

Responsibilities

• Conduct comprehensive assessments of participants’ basic needs, living situations, mental health status, academic levels, and employment readiness.
• Provide individualized case management and develop intervention, treatment, and educational plans aligned with program guidelines.
• Offer job coaching and accountability support, guiding participants from training to employment stability and collaborating with worksite supervisors.
• Monitor participant progress, maintain accurate case files, and complete all required reports and documentation on time.
• Coordinate resources for housing, food, clothing, healthcare, and other essential needs while building and maintaining referral networks.
• Facilitate job training classes and support participants in achieving career-relevant certifications.
• Respond to Transitional Housing support calls, conduct safety assessments, transport clients to key appointments, and assist with apartment walk-throughs and setup.
• Partner with Outreach Employment Center (OEC) and Transitional Housing (TH) leadership to enhance program quality, strengthen community partnerships, and contribute to continuous improvement efforts.
• Attend weekly team meetings and collaborate with cross-department staff to coordinate holistic services for clients.
• Maintain administrative accuracy, manage receipts and documentation, and submit program statistics for organizational oversight.
• Represent the organization with professionalism and sensitivity when interacting with diverse populations, including individuals in crisis or transition.
  
  

Requirements

• Master’s degree in social work, psychology, or related field; pre-licensure required with supervision hours available or licensure preferred.
• Minimum of three years of case management or social services experience, ideally within housing, employment, or transitional programs.
• Experience working with youth or young adults from low-income, high-risk, or culturally diverse backgrounds.
• Basic proficiency in Microsoft Word and Excel.
• Valid driver’s license, clean driving record, and reliable transportation required.
• A commitment to Outreach Community Ministries’ Statement of Mission and Faith is required.
