What are the responsibilities and job description for the Care Coordinator - Medical position at Outpatient Medical Center, Inc.?
ESSENTIAL FUNCTIONS
The overall responsibilities of the Care Coordinator include, but are not limited to, making specialty and diagnostic referrals according to referral procedures. Contact patients on the HMO list provided to us, verify the need for care, and schedule an appointment accordingly. Contact and make appointments for patients that are due for wellness visits based on directives from Executive Officers. Work with the DON or CMO regarding medical questions and clinical priorities. Report daily on outcomes of contacts and appointments made. Follow up with patients for whom we need medical records or immunization information. Attend assigned meetings as directed. Register and schedule follow-ups for patients as needed.
KNOWLEDGE, SKILLS AND ABILITIES
Knowledge to understand directives from Executive Officers in a manner that maximizes the performance of the various departments.
Skill in written and verbal communication with Executive Officers and other staff; maintain an organized work area with attention to detail; organize and prioritize workload and meeting deadlines.
Ability to interact effectively and professionally with persons from diverse cultural, socioeconomic, education, racial, ethnic and professional backgrounds. Ability to determine if a patient is fully registered or not and get them registered if needed; Be able to schedule work day/ work week based on directions from Executive Officers; Ability to work as an effective team member; Ability to schedule according to the approved protocol Consistently maintains ethical behaviors, maintain confidentiality and works toward goals and objectives of Strategic Health Plan. Follow OMC policies and the mission, vision, receive constructive feedback; and be available to meet disaster-related OMC responsibilities. Proficient use of basic office software including, Microsoft Word, Excel, and PowerPoint.
EDUCATION AND EXPERIENCE
Preferred: OMC prefers candidates with one year experience with medical referrals or dental care coordination or behavioral health case management
Required: Experience in documenting in electronic medical records systems and in collaborative communications with outside referral entities, providers, and patients.
LICENSE and /or CERTIFICATION
Driver's license
SPECIAL REQUIREMENTS
Must have reliable transportation and be willing to travel between OMC facilities if needed.
