What are the responsibilities and job description for the Psychiatric-Mental Health Nurse Practitioner (PMHNP) position at Outpatient Addiction Clinic?
We are seeking a Psychiatric-Mental Health Nurse Practitioner (PMHNP) to join our organization.
We provide full service outpatient addiction treatment including but not limited to; primary care evaluation/follow-up, MAT, individual and group counseling, peer recovery support, case management, mental healthcare, nursing and lab services, and HCV treatment referral.
The position is part-time and a combination of in-person and telehealth preferred but fully telehealth also a possibility for the right candidate. A collaborating physician relationship provided.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provide initial evaluation, diagnosis, treatment and ongoing support for individuals with substance use and co-occurring mental health disorders
- Complete full and appropriate documentation to support professional billing compliance
- Provide education regarding illness and prognosis to assist with clarification of patient goals of care and support development of a comprehensive integrated care plan
- Interpret test results to identify any health issues and provide feedback to patients
- Educate and counsel patients regarding medication compliance
- Work with an interdisciplinary team of professionals to provide integrated care for our patients.
- Patient follow-up visits for primary and addiction care.
- Collaborate with other patient providers for high quality care.
- Evaluate lab results and provide consultation/education
- Review and oversee progress and care plans, including making any adjustments
- Maintain required licensure, certifications and CME credits
MINIMUM QUALIFICATIONS:
- Active and unrestricted medical license as a certified nurse practitioner within the state of Ohio.
- Experience providing psychiatric/addiction clinical services for both medically and socially complex populations; Experience managing complex mental health and substance use disorders
- Ability to develop relationships with network and community providers.
- Current DEA licensure and prescriptive authority in Ohio
- Ability to work on-site and or remotely with diverse care teams in a variety of settings
- Organized and outstanding communication skill
- Technologically competent and experienced with Electronic Health Records
- Must have a high level understanding and commitment to excellent customer service
