Join our team as an MDS Coordinator in a dynamic, collaborative environment where you’ll play a key role in ensuring timely and accurate completion of the RAI process and care management from admission to discharge.
What You’ll Do:
- Collaborate with the Interdisciplinary Team to assess resident needs and create MDS assessments and care plans in accordance with state and federal guidelines.
- Ensure accurate and timely documentation of the MDS process, including coding, medical record reviews, and interdisciplinary updates.
- Assist in coordinating daily stand-up meetings and review care and assessment schedules.
- Stay updated on federal, state, and certification guidelines to ensure compliance with all regulations.
- Submit resident data to state and federal agencies, and ensure all MDS documentation is accurate and complete.
- Graduate of an approved Registered Nurse (RN) or Licensed Practical Nursing (LPN) program and licensed in the state of practice.
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility (preferred).
- In-depth knowledge of the MDS 3.0, Medicare PPS process, Medicaid reimbursement, and OBRA regulations.
- Experience with care planning and quality indicators.
