A Day in the Life of this Role:

Reporting to the Sexual Health Program Manager, the Sexual Health Nurse Navigator will primarily be responsible for supporting the Sexual Health Programs by empowering, educating, and supporting individuals receiving treatment, prevention, and education services.

Summary of Duties:

  • Provides guidance, counseling and advocacy to clients in need of assistance in accessing HIV treatment, Hepatitis C treatment, HIV Pre-Exposure Prophylaxis (PrEP), Non-Occupation Post Exposure Prophylaxis (nPEP), Sexually Transmitted Infections (STI) testing and treatment.
  • Provides counseling and education on Standards of Care for patients for HIV treatment/prevention, Hepatitis C Treatment, and STIs.
  • Provide guidance and assistance in linking to other care such as Primary care, Behavioral Health care, Vision care, and Dental care.
  • Assist patients in referral and registry for Drug Assistance Programs and other discount programs.
  • Coordinate client linkage to Insurance coverage and/or Sliding Fee Scale Discount program.
  • Maintains concise and accurate documentation according to departmental standards including but not limited to chart notes, access timelines, refill timelines, and Case Management forms from the Massachusetts Department of Public Health (MDPH).
  • Facilitates timely scheduling to ensure proper treatment and follow-up. Collaborates with Program Manager and MDPH for proper reporting and tracking.
  • Collaborate with other organizations to ensure proper treatment and follow-up for patients
  • Reviews and monitors Continuous Quality Improvement (CQI) with Sexual Health Program Manager to evaluate programming ensuring achievement of contractual and regulatory requirements.
  • Ability to provide client-centered HIV/STI counseling services in a stigma-free environment.
  • Facilitates and renders treatment and immunizations as ordered by providers and or protocol. Reviews treatment plan with patient while documenting patient education and patient’s understanding of treatment, and prevention of future infections.
  • Tracks and documents patient follow-ups and barriers to follow-up.
  • Knowledge of HIV/AIDS/STI prevention, transmission, and epidemiology, or willingness to learn.
  • Phlebotomy certification/experience or a willingness to be trained in phlebotomy.
  • Experience working with diverse groups regardless of race/ethnicity, sexual orientation, gender identity, sex and drug cultures, age groups, religion, country of origin, disability, and economic status.
  • Ability to multi-task on a daily basis.
  • Proficiency with use of computers and software including but not limited to Microsoft Excel, Word, PowerPoint, and Electronic Medical Record (EMT), email, and office equipment.
  • Ability to work in a multi-disciplinary, collaborative environment.
  • Excellent verbal and written communication skills including ability to talk openly about sex, sexual risk-taking behaviors, and harm reduction strategies.
  • Strong attention to detail and data collection.
  • Ability to work weekends and some evenings.
  • Ability to attend required MDPH trainings in Massachusetts.
  • Option to attend other trainings/conferences as available/desired.
  • Other related duties and projects assigned by the Program Manager.

What we need from you:

  • Registered Nurse (RN) current licensure in the Commonwealth of Massachusetts
  • Nursing Education Programs must be accredited by American Association of Colleges of Nursing (AACN).
  • Experience, knowledge and education related to the challenges, barriers and needs of the diverse HIV population, particularly the LGBTQ community, and existing HIV/AIDS resources
  • Working knowledge of current Sexual Health initiatives, or willingness to learn
  • Excellent interpersonal skills, communication skills, organizational abilities, computer experience, and the ability to work within a collaborative team setting.
  • Excellent written and verbal communication skills – bi-lingual helpful
  • Proficiency in computer skills including EPIC, Outlook, Microsoft office, OneDrive etc.
  • Excellent judgment and ability to solve problems in a timely manner.
  • Ability to adhere to strict confidentiality standards.
  • Demonstrated ability to work effectively both as a team member and independently.
  • Demonstrated ability to work in a culturally diverse and inclusive environment
