Job Overview
We are seeking a compassionate and dedicated Social Worker to join our up and coming Hospice team. The ideal candidate will have a strong background in providing support and counseling to individuals facing various challenges, including those related to geriatrics, autism, developmental disabilities, and mental health. This role requires a proactive approach to patient assessment and the ability to implement effective therapeutic strategies. As a Social Worker, you will play a crucial role in improving the quality of life for your clients through individualized care plans and support.
Responsibilities
- Conduct comprehensive patient assessments to identify needs and develop appropriate care plans.
- Provide psychotherapy and mental health counseling to individuals and families.
- Utilize motivational interviewing techniques to encourage positive behavioral changes.
- Offer counseling services focused on child and family dynamics.
- Work collaboratively with multidisciplinary teams in clinical settings to ensure holistic care.
- Collect and analyze data related to client progress and treatment outcomes.
- Advocate for clients' needs within the healthcare system and community resources.
- Maintain accurate documentation of client interactions, treatment plans, and progress notes.
Requirements
- A Master's degree in Social Work (MSW) from an accredited institution is required.
- Valid state licensure as a Social Worker (LCSW or equivalent).
- Experience working with geriatrics, individuals with autism, or those with developmental disabilities is preferred.
- Strong skills in psychotherapy, patient assessment, and motivational interviewing.
- Familiarity with child family counseling practices is advantageous.
- Excellent communication skills, both verbal and written.
- Ability to work independently as well as part of a collaborative team environment.
