Duties
- Conduct comprehensive patient assessments to determine eligibility for hospice services and develop individualized care plans aligned with patient needs and goals
- Manage and coordinate clinical case management activities, including symptom management, medication administration, and monitoring of patient progress
- Collaborate effectively with healthcare professionals across disciplines such as physicians, social workers, chaplains, and therapists to ensure holistic patient care
- Provide education and emotional support to patients and families regarding disease progression, treatment options, and end-of-life planning
- Oversee discharge planning and coordinate transitions between inpatient, outpatient, home health, and post-acute care settings
- Maintain accurate medical records using Electronic Health Records (EHR) systems while adhering to HIPAA regulations to ensure patient privacy and data security
- Participate in quality improvement initiatives and ensure compliance with health regulation policies related to hospice care standards
Requirements
- Current RN licensure with active state registration; prior experience in hospice or palliative care preferred
- Critical care experience including ICU, Emergency Room, or Medical-Surgical units; Level I Trauma Center or Level II Trauma Center experience is highly desirable
- Knowledge of medical terminology, physiology, and disease processes relevant to end-of-life care
- Demonstrated ability to perform thorough patient assessments and manage complex clinical cases effectively
- Proficiency in Electronic Health Records (EHR) management and familiarity with healthcare technology tools such as EMR/EHR systems
- Strong communication skills for client interaction and interdisciplinary collaboration
- Geriatrics, behavioral health, infectious disease care, or post-acute care experience is a plus
- Ability to manage multiple priorities efficiently while maintaining compassionate patient service standards
- Knowledge of health regulation policies including HIPAA compliance and medical records management

Pay: $70,000.00 - $80,000.00 per year
