Our Hospice is looking for a Nurse Practitioner (NP). You will be responsible for applying nursing knowledge at an advanced level to maintain or improve quality of life of seriously ill patients in the home, inpatient or outpatient setting through specialized clinical management of identified symptoms. The role serves as a consultant to referring physicians to assist managing patient's symptoms and providing goals of care conversations with patients and families. He/she will also serve as a resource, consultant, and advisor to the clinical staff and leadership to ensure that our patients receive the highest quality of care.

• Hours: Full-time, 4 day per week, 40 hours per week (in-person)
  
  

A Nurse Practitioner at Our Hospice Palliative Care will:

• Applies nursing knowledge at an advanced level to maintain or improve quality of life for seriously ill patients.
• Performs consultative, collaborative, supportive visits primarily in the outpatient or inpatient settings, to address symptom management and/or goals of care for patients with an advanced serious illness.
• Travels to the patient location whether it is in the home, nursing home, or physician office when needed to conduct visits.
• Assists the Palliative Office Manager in the continued development and implementation of the Palliative Support Center's specialty clinic.
• Coordinates outpatient clinic activity.
• Conducts focused medical assessments to include history & physicals; evaluates diagnostics; interprets findings and provides recommendations to the referring physician.
• Provides diagnostic & pharmacologic management of acute and chronic disease processes through ongoing assessment, intervention and education in collaboration with the physician.
• Practices within the guidelines established by the protocols, policies and procedures of the Palliative Support Center.
• Provides interventions to enhance the quality of life of patients and families through comprehensive assessment and treatment of pain and other symptoms.
• Provides symptom management and/or recommendations to the referring physicians based on assessment findings and patient and family goals.
• Collaborates with the clinical and interdisciplinary groups involved in care and the Palliative Support team regarding clinical findings and coordinating the plan of care.
• Consults with medical director regarding patient care issues beyond the realm of experience or autonomy of the Advanced Practice Nurse.
• Identifies and addresses educational needs of the patient and family.
• Provides education related to health care choices for the patient and family and advocates effectively for patient comfort, dignity and choice.
• Identifies and addresses the need for Advanced Directives including Do Not Resuscitate (DNR), Living Will and Health Care Power of Attorney.
• Coordinates services with the patient's referring Physician, Specialists and hospitals as necessary to promote patient and family satisfaction as well as continuity of care across the health system.
• Documents communication, findings, care provided and outcomes per palliative care standards.
• Participates in the Palliative Support team meetings.
• Participates in call rotations.
• Assists the Medical Director as requested with hospice coverage.
• Participates in the data collection process for defined key metrics for continued program development and success.
• Promotes palliative care and provides education to all potential referring customers including but not limited to patients and family members, case managers, nursing facilities administrators and staff, physicians and physician office staff, and community groups.
• Provides education related to palliative care guidelines, processes and provision of care to Our Hospice staff as needed.
• Performs additional duties and/or other responsibilities (as deemed necessary by Palliative Care Manager and/or Medical Director).

An Nurse Practitioner at Our Hospice Needs:

• MSN from accredited program
• Valid APRN License in the State of Indiana
• Valid Indiana drivers license
• Preferred to have experience in hospice and palliative medicine
• Preferred to have experience working as a provider in a medical clinic
