Position Title: Registered Nurse (RN), Case Manager

Position Summary: Case Management: Responsible for the clinical management and coordination of skilled nursing services for patients admitted to Oswego Health Home Care. This role includes conducting comprehensive assessments, developing and updating individualized plans of care, and overseeing the delivery of skilled services in accordance with provider orders. The RN Case Manager provides ongoing evaluation of the patient status, manages care transitions and collaborates with the interdisciplinary team to ensure quality outcomes, regulatory compliance, and continuity of care.
Education, Training, Experience, Certification and Licensure: Licensed professional nurse with a minimum of 2 years’ clinical experience in a hospital setting. Utilization review, clinical record and/or managed case experience desired. Participates in orientation and continuing education and updates and maintains knowledge and skills related to specific areas of practice. Completes agency wide annual competencies as identified in scope of care and services.
Special Equipment, Skills, or Other Requirements: Strong clinical assessment, performance of high tech procedures, care coordination, critical-thinking and leadership skills. Competent in BLS/CPR rescues. Oral and written communication skills. Demonstrated ability for problem solving and conflict management. Familiarity with Microsoft Word, Microsoft 365, electronic health records and clinical documentation.
Work Environment and Hazards: OSHA category I. Routine or potential exposure to blood, body fluids, excretion or secretions. Exposure to communicable disease, traveling in community at large, driving in inclimate weather. Exposure to outdoor climate, tobacco smoke, household pets. Office Environment is Exposure class II.
Physical Demands: Moderate work: lifting, bending, stooping, pushing, pulling and ability to climb stairs. Heavy work: standing and walking. Some repetitive motion of eyes and wrists with writing or data entry. Light work: sitting, standing, walking, lifting, pulling, pushing, and carrying. Exposure to a CRT and repetitive motions (data entry)

Pay Range: $35.00-$41.20/hr
Employee salary is based on commensurate experience and other qualifications.
