POSITION SUMMARY: The OSF OnCall Connect APN (APN) is a member of a collaborative team who is responsible for providing virtual care to a diverse population of patients across the Ministry through technology in their home environment. The APN will perform a variety of functions including assessment, diagnosis, and intervention within their scope of practice. Utilizes evidenced based care and clinical decision support to guide practice. Performs ongoing communication and works to promote quality of care through collaboration with physicians, advanced practice providers, and other care team members, patients and families. The APN demonstrates service excellence and accountability through recognizing the direct relationship between cost-effective and efficient provision of care and services including the impact of access to care.
Qualifications
REQUIRED QUALIFICATIONS: Education: Master’s degree in nursing Experience: 2 years of clinical experience Licensure/ Certification: Must have an Illinois RN License and Illinois APN license upon hire. A Michigan RN license and a Michigan APN license must be obtained within 6 months of hire. Other Skills/ Knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem-solving skills, with the ability to be detail oriented. Excellent communication skills, including telephone/video etiquette and customer service. Understanding of the principles of performance improvement, case management, discharge planning, utilization review, and medical records. Demonstrated ability to work independently. PREFERRED QUALIFICATIONS: Experience: Advanced Practice Provider Working knowledge of Epic EMR System
