We are currently seeking a Program Manager. The Program Manager will coordinate the day-to-day operations of two key initiatives: the Corrections Health Initiative (CHI) program and the Queensboro Family Focused Discharge Planning program. This role ensures the successful achievement of goals and objectives across both programs while maintaining superior relations with program constituencies and fostering collaborative partnerships among staff.


For the CHI program, the manager will manage operations within Sing Sing, Edgecombe, and Queensboro Correctional Facilities, as well as the Bronx. This includes providing education on HIV/HCV, STIs, sexual health, reentry, and overall wellness; creating transitional plans for individuals with HIV/HCV; and providing case management for people released to the Bronx.


For the Queensboro program, the manager will oversee family-focused discharge planning, developing plans based on participant and family needs to ensure successful reentry and family functioning.


The Program Manager will supervise staff across both programs, including 2 Linkage Specialists and 2 Family Services Specialists, and will directly provide trainings to Peer Trainees inside prisons. A clearance to work within NYS Correctional Facilities, including a background check, is required and must be maintained.


Salary: $70,000 - $85,000 annually 

Requirements

 Essential Duties:

  • Collaborate with the Vice President of Prison Services to establish contract goals, objectives, and work plans for both programs.
  • Supervise and assist in career development for direct reports, including 2 Linkage Specialists and 2 Family Service Specialists, by providing supervision and identifying training opportunities.
  • Implement work activities to meet goals; monitor and evaluate the effectiveness of results.
  • Ensure all contract deliverables are met.
  • Oversee, promote, conduct, and coordinate health education groups on topics including HIV/HCV, STIs, sexual health, and wellness.
  • Conduct and supervise peer trainings in prisons and keep records of training activities.
  • Develop family-focused discharge plans based on participant and family needs and provide appropriate community referrals.
  • Provide outreach to potential participants within correctional facilities, screen them for program enrollment, and orient them to program objectives.
  • Provide direct services as needed, including visiting prison sites several times a week.
  • In collaboration with the Data Analyst, ensure that record keeping and reports are accurate and timely.
  • Actively engage with funders and referral sources to maintain positive working relationships.
  • Ensure a welcoming environment for all participants.
  • Coordinate with correctional health staff to recruit participants and obtain medical documentation.
  • Keep abreast of the latest information about HIV/AIDS, Hepatitis, and other co-infections.
  • Attend and participate in case conferences with Parole.
  • Establish and maintain contacts with community-based organizations.
  • Adhere to established DOCCS, AIDS Institute, and Osborne guidelines for operating prison and community reentry programs.
  • In-person attendance is required to collaborate with co-workers.
  • Perform other duties as assigned.

Minimum Qualifications:

  • Associate Degree in Human Services, Public Administration, Criminal Justice, Social Work or a related field.
  • Two years of experience supporting a non-profit organization in a supervisory capacity.
  • Knowledge of the criminal justice environment, transitional planning, or HIV/HCV.

  • Ability to obtain and maintain clearance to work in New York State prisons.
  • Ability to travel locally and to assigned New York State prison facilities.
  • Competence within the criminal justice environment and social service processes.
  • Excellent assessment, treatment planning and conflict resolution skills.
  • Effective oral and written communication skills.
  • Ability to analyze and integrate information independently.
  • Exceptional database management skills.
  • Proficiency in Microsoft Office and/or Google Suite.
  • Ability to work effectively under tight deadlines, pay strong attention to detail, and time management skills.
  • Ability to work in teams and collaborate effectively with people in different functions.
  • Ability to take the initiative and drive for results.
  • Ability to communicate with all levels of the organization and build strong relationships.
