Job Summary: The Outreach Worker promotes the services and mission of Osborn Family Health Center through community engagement activities.
Specialized Skills: Experience and understanding of community culture, and health needs is important. Excellent interpersonal and group communication skills necessary.
Summary of Accountabilities:
- Demonstrates the mission, vision, and values of Osborn Family Health Center in behaviors, practices, and decisions.
- Serves as a patient advocate in direct interactions with the patient, care team, and community partners.
- Demonstrate organizational skills in planning, implementing, and reporting community event projects.
- Provides the most up to date community resources utilizing technology and resource guides that are culturally sensitive and appropriate to the literacy level of the patient at community events.
- Promotes the mission, services, and staff at community events in a positive manner with the goal of connecting consumers to primary care.
- Identifies as an Outreach Worker representing Osborn Family Health Center and conducts oneself accordingly as is evidenced in demeanor in behavior, appearance, and communication.
- Utilizes supervision and education opportunities, to engage in career long learning.
- Practices personal reflection and self-correction to assure continual professional development and awareness of professional roles and boundaries.
- Provides leadership in promoting sustainable changes in service delivery and practice to improve the quality of services and the health care center services.
- Understands and is able to verbalize annual performance improvement measures and will assist with performance improvement opportunities by identifying and closing gaps in care.
- Actively participates in departmental meetings, committees, teams, huddles, and assists in quality improvement activities.
- Acts as liaison for Osborn Family Health Center in all community affairs serving as an advocate for human rights and social and economic justice.
- Promptly and accurately responds to any type of patient inquiry and uses effective problem solving strategies for patient satisfaction.
- Maintains strict HIPPA compliance at all times according to Osborn Family Health Center policy.
- Responds to families, visitors, staff, and community partners in a sensitive empathetic manner.
- Cultivates positive relationships with all patients, customers, community partners, and members of the care team.
- Assists with any additional duties as needed or as directed by Director of Social Work and Leadership team.
- Assists in the coordination of patient, healthcare center, and community focused projects and takes initiative to create new and innovative work flow.
Minimum Certifications, Registry and/or Licenses Required:
Health Worker training or certification preferred.
Minimum Degree and Experience Required:
High School diploma. College recommended.
Skill Requirements:
Skill in establishing and maintaining effective working relationships with other employees, patients, hospital departments, organizations and the public. Strong interpersonal and communication skills. Knowledge of basic computer skills. Project coordination and community liaison experience preferred. Must be able to work independently.
