Key Responsibilities:
- Conduct thorough physical therapy evaluations and develop appropriate treatment plans.
- Deliver evidence-based PT interventions tailored to each resident’s needs and goals.
- Monitor and document patient progress and adjust care plans as necessary.
- Communicate effectively with interdisciplinary team members, residents, and their families.
- Ensure accurate and timely clinical documentation in compliance with facility and regulatory standards.
- Current and valid Physical Therapist license in the state of PA.
- Graduate of an accredited Physical Therapy program.
- Previous experience in a skilled nursing or long-term care setting is preferred.
- Strong clinical skills and a passion for geriatric care.
- Ability to work independently and adapt to a dynamic environment.
- Competitive hourly rate of $55–$60, commensurate with experience
