Role Description
We are seeking a dedicated Physical Therapist or Physical Therapy Assistant for a full-time, on-site position in one of our Northern IL facilities. In this role, you will provide comprehensive physical therapy services tailored to meet the specific needs of each patient. Responsibilities include managing patient conditions, creating and implementing individualized treatment plans, and providing ongoing rehabilitation care. Collaboration with other healthcare professionals to achieve optimal patient outcomes is an essential component of this role.
Qualifications
- Strong foundation in Physical Therapy, Rehabilitation, and Patient Care
- Proficiency in Medicine and knowledge of best practices in therapeutic interventions
- Experience in both clinic-based treatment and Home Care environments
- Excellent communication and interpersonal skills to support and motivate patients
- Active state licensure in Physical Therapy
- Ability to work collaboratively with a multidisciplinary team
- Experience in orthopedic or sports injury rehabilitation is a plus
