Job Purpose:

The Health History Specialist is responsible for completing accurate and thorough collection of patient health histories prior to their scheduled appointments. This role ensures providers receive essential clinical information in advance, supports efficient clinic workflow, enhances patient experience, and promotes safe, informed, and timely care. The position requires strong communication skills, attention to detail, and the ability to obtain sensitive health information with professionalism and confidentiality.

 

Primary Duties and Responsibilities:

• Communicates courteously and professionally with patients to explain the purpose of pre-visit history collection.

• Obtains patient triage information as well as any necessary reports, diagnostic records for physician review and completes patient follow-up steps based on provider protocols. 

• Handles image reports and records and forwards to appropriate surgeon for review  

• Supports in the coordination of patient appointments.

• Serves as a liaison between multidisciplinary departments and provider teams to appropriately direct patients to receive proper care.  

• Captures complete and accurate patient information for referrals. 

• Obtains medical records and reviews for accuracy.

• Ensures all patient health histories are completed within 14 days of appointment, as applicable.

• Assists the team with gathering history and physicals, applicable testing, and communicating with patients about additional appointment needs.

• Adheres to all policies and procedures established by facility leadership and provides thorough patient instructions. 

• Maintains patient privacy and comply with HIPAA regulations.

• Other duties as assigned.

Education and Experience Requirements:

• High school diploma or GED required

• Minimum 1 year of customer service experience required

• Working knowledge of Microsoft and Google Suites

• Previous clinical experience preferred

 

Environmental/Working Conditions: 

Working environment is in an ambulatory surgery center setting.

 

Physical/Mental Demands:  

• Sit, stand and walk for extended periods of time

• Requires lifting, carrying and/or moving objects in a manner consistent with most office environments (generally, no more than 10 pounds on a frequent basis and 20 pounds on an occasional basis)

• Possess full range of motion in shoulders and arms, including reaching above or below the shoulder

• Finger dexterity for typing on a keyboard and using a mouse for extended periods of time; ability to twist hands or wrists repetitively

• Good balance. 

• Vision (e.g., depth perception, color vision, strong vision up close); hearing (e.g., high pitch sounds, soft or distant sounds)
