Job Summary
We are seeking a dedicated Physical Therapist (PT) to join our home health team serving Orleans and northern Essex counties of Vermont. In this role, you will provide personalized, evidence-based care to patients in their homes, helping improve mobility, reduce pain, and enhance overall quality of life.
This position offers flexibility, autonomy, and the opportunity to work with a diverse patient population, including pediatric and geriatric patients.
Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide hands-on therapy including orthopedic rehab and trigger point therapy
- Deliver care to patients across various age groups
- Coordinate with healthcare teams on care plans and discharge planning
- Document patient progress using EMR systems
- Educate patients and families on exercises, recovery plans, and injury prevention
- Adjust treatment plans based on patient progress and needs
Requirements
- Active Physical Therapist license in Vermont (required)
- Experience in home health, outpatient, or inpatient settings
- Strong knowledge of anatomy, physiology, and rehabilitation techniques
- Experience with EMR documentation systems
- Excellent communication and patient education skills
- Ability to work independently and manage time effectively
- Reliable transportation to travel within service area
Preferred Qualifications
- Home health experience
- Experience treating pediatric and geriatric patients
- Familiarity with discharge planning and multidisciplinary care coordination
Pay: $34.75 - $56.74 per hour
