What are the responsibilities and job description for the MDS Coordinator position at Orem Rehabilitation & Skilled Nursing?
We are seeking a detail-oriented MDS Nurse (RN or LPN) to join our interdisciplinary team. The MDS Nurse is responsible for coordinating and completing resident assessments in accordance with CMS and state guidelines, ensuring accurate clinical documentation, timely MDS submissions, and appropriate reimbursement under PDPM. This role works closely with nursing, therapy, and other departments to support individualized care planning, regulatory compliance, and quality outcomes for our residents. The ideal candidate is organized, collaborative, and passionate about ensuring residents’ needs are accurately represented through thorough assessment and documentation.

Job Responsibilities:
- Primary role is to conduct and coordinate the Resident Assessment Instrument (RAI) that is the Minimum Data Set (MDS).
- Conduct Care Assessments (CAAs) and Care Plans in accordance with State and Federal requirements.
- The MDS nurse participates as part of the facilities Interdisciplinary Team (IDT) to manage patients care.
- Other nursing duties as apply.
Requirements:
- Current Registered Nurse (RN) or Licensed Vocational Nurse (LVN/LPN) license in the state of Utah.
- Experience working in the skilled nursing/ long term care setting preferred
- Strong communication and inter-personal skills.
- Must work well as part of an interdisciplinary team.
