Job Summary:
We are seeking a Support Coordinator to join our team. As a Support Coordinator, you will be responsible for providing support to individuals with developmental disabilities.
Responsibilities:
· MANAGES A CASELOAD OF MODERATE SIZE AND COMPLEXITY INVOLVING INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES.
· BUILDS AND MAINTAINS STRONG, TRUSTING, PROFESSIONAL RELATIONSHIPS WITH CLIENT’S SERVED AND OTHERS INVOLVED WITH THAT PERSON.
· PREPARES CASE PROGRESS NOTES AND OTHER REPORTS CONCERNING THE CLIENT’S CARE.
· DEVELOPS, IN COOPERATION WITH OTHER STAFF, AN INDIVIDUAL SUPPORT PLAN FOR EACH CLIENT.
· ATTENDS INTER-DISCIPLINARY TEAM MEETINGS TO DISCUSS CLIENT CASES IN RELATION TO ELIGIBILITY, SUPPORT PLANS, PROGRESS, AND POSSIBLE CHANGES IN THE SERVICE PROGRAM.
· MATCHES THE UNIQUE SUPPORT NEEDS OF INDIVIDUALS AND FAMILIES WITH RESOURCES IN THE COMMUNITY.
· PREPARES PURCHASE OF SERVICE AND WAIVER PARTICIPATION AUTHORIZATIONS AND ARRANGES FOR CLIENTS TO ACCESS SERVICES.
· MONITORS SERVICES TO ENSURE THE TERMS OF AUTHORIZATION ARE BEING FULFILLED BY VENDOR, CHECK ON QUALITY OF SERVICES, AND REVIEW CLIENT PROGRESS.
· CONDUCTS CLIENT ASSESSMENTS INVOLVING THE USE OF SPECIALIZED KNOWLEDGE AND APPLICATIONS.
· PERFORMS OTHER RELATED WORK AS ASSIGNED.
Knowledge, Skills, and Abilities:
· INTRODUCTORY KNOWLEDGE OF CASE MANAGEMENT METHODS, PRINCIPLES, AND TECHNIQUES.
· INTRODUCTORY KNOWLEDGE OF VARIOUS DEVELOPMENTAL DISABILITIES
· ABILITY TO MANAGE A CASELOAD OF CLIENTS WITH DEVELOPMENTAL DISABILIITIES, KEEP SUPPORT PLANS CURRENT, AND TO MAINTAIN ACCURATE RECORDS.
· ABLITY TO COLLECT AND ANALYZE INFORMATION TO MAKE DECISIONS CONCERNING A CLIENT’S SUPPORT PLAN.
· ABILITY TO DEVELOP A LOGICAL, FEASIBLE, AND PRACTICAL SUPPORT PLAN FOR CLIENTS WITH DEVELOPMENTAL DISABILITIES.
· ABILITY TO EVALUATE THE PROGRESS OF CLIENTS AND THE QUALITY OF THEIR SERVICE PROGRAMS.
· ABILITY TO COMMUNICATE EFFECTIVELY.

1. Any candidate seeking employment as a Support Coordinator must, minimally, meet the following educational standards:
a. A Bachelor’s Degree from an accredited college or university.
b. A Registered Nurse license
2. Along with minimum qualifications Missouri has adopted Support Coordinator Core Competencies to ensure person-centered, consistent and quality support coordination is received across the state. A copy of Support Coordinator Core Competencies will be provided at the time of interview and will be attached to the policy manual.
