Job Summary
We are seeking compassionate and dedicated Caregivers to provide exceptional in-home and community-based support to individuals with diverse needs. The ideal candidate will assist with daily living activities, monitor patient health, and promote independence while ensuring safety and comfort. We have a 12 hour night shift available for Saturday nights 8pm to 8am in Medford.
Pay $19 Per hour
Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance
- Provide medication administration and monitor vital signs in accordance with care plans
- Support patients during meal preparation and feeding, ensuring nutritional needs are met
- Observe and document patient condition, behavior, and progress for care planning and reporting purposes
- Maintain a clean and safe environment through light cleaning and sanitation practices
- Implement specialized care protocols for residents with dementia, Alzheimer's, autism, or other disabilities
- Assist with transportation to appointments or community activities as needed
- Follow HIPAA regulations to ensure patient privacy and confidentiality at all times
- Collaborate with healthcare professionals to deliver comprehensive patient-centered care
Skills
- Prior experience in assisted living, home health, hospice care, or long-term care settings preferred
- Knowledge of patient monitoring, vital signs measurement, and medication administration
- Certification in CPR and First Aid required; additional certifications such as HIPAA training are a plus
- Experience working with individuals with developmental disabilities, autism, dementia, or memory care needs highly desirable
- Strong observational skills for patient safety and condition monitoring
- Ability to work independently and as part of a team in various settings including group homes or community environments
- Compassionate communication skills tailored to diverse populations including seniors and individuals with disabilities
- Physical ability for heavy lifting, assisting with transfers using Hoyer lifts, and supporting mobility needs
- Familiarity with care plans and documentation procedures related to home & community care
