What are the responsibilities and job description for the Community Habilitation Specialist position at OPWDD Self-Direction?
Seeking a Com Hab staff for a young man in Greenlawn. He is very high functioning and is primarily diagnosed with Autism. He lives with family, and is gainfully employed part-time. He is very sweet and easy going, and would like staff to assist him with accompanying him to classes/programs he is involved in and finding other ways to enjoy his interests.
Hours needed are Tuesday & Thursday starting around 5PM. Friday's also available same time, if desired.
AGENCY/SALARY - AIM Services, pay starts at Minimum $23/hr - commensurate on experience
Schedule: Tues & Thursdays 5PM.
Job Title: Community Habilitation is a Medicaid-funded program operated under OPWDD to provide one-to-one training to people with intellectual/developmental disabilities to develop or enhance the skills needed to live more independently in their homes or in the community.
AGENCY: Will have to be hired with AIM Services, Inc.

Must be 18 , have a clean, valid driver's license & reliable transportation. Will have to be fingerprinted upon hire. Will have to use personal vehicle to bring participant out into the community. Will be paid transportation and activity reimbursement.
