Overview
The Case Manager-Skills Trainer provides both recovery-focused and psychosocial services to people with severe and persistent mental illness and/or substance use disorders, or to other program-specific populations. Provides outreach and engagement services and connects people to needed resources and treatment. Organizes and leads activities, including individual and group skills training, designed to enhance and promote social connectedness, symptom management, recovery and effective life skills of adults and families. Coordinates with other community agencies providing services to clients. The Case Manager/Skills Trainer interviews and screens clients, and helps formulate and carry out individual treatment plans. Assists clients with accessing local, state and federal resources and entitlements. Services are primarily delivered in the clients’ natural environment.
Responsibilities
Case Manager/Skills Trainer helps formulate and carry out individual treatment plans, working in partnership with other team members and community partners. Assists clients with accessing local services and supports such as housing, social security benefits, education, physical health care etc. Provides active outreach and engagement to promote engagement in needed treatment services and community supports. Teaches life skills such as money management, emotional self-regulation, good interpersonal skills, self-advocacy and utilizing public transportation. Liaisons with community partners providing education on Options behavioral health services and facilitating access to these services. The majority of services are delivered in the community. Regularly drives agency vehicle and transports clients as required. Manages time and caseload effectively.
Travels between sites and is required to drive.
Qualifications
Bachelor’s (BA/BS) degree in a human services field OR three years of experience providing services in a similar setting with individuals who have emotional and/or mental and/or developmental disabilities, OR a combination of education and experience that provides the desired skills, knowledge, and abilities required to perform the job.
Must be a QMHA on the Mental Health & Addictions Certification Board of Oregon (MHACBO) registry.
