Duties
- Assess patients’ physical, cognitive, and emotional abilities to develop tailored treatment plans(Plan of Care) that promote optimal functioning.
- Assist patients with Activities of Daily Living (ADLs), such as dressing, grooming, and mobility, to support independence.
- Conduct comprehensive evaluations and assessments of patients' conditions, abilities, and functional limitations
- Educate patients and families on proper use of equipment and techniques to enhance their progress
- Maintain accurate and complete patient documentation
- Provide hands-on therapy and rehabilitation services in the home environment
- Develop and implement individualized treatment plans and goals to meet patients' needs and help them reach their maximum level of independence
- Engage in interdisciplinary teamwork to deliver comprehensive care.
- Ability to handle physically demanding tasks and work effectively in various home environments
Requirements
- Valid occupational therapy license in the relevant jurisdiction.
- Experience in home health or community-based setting
- Ability to work independently and manage a flexible schedule and caseload of patients
- Experience in educating and training caregivers and family members.

Pay: $65.00 - $95.00 per hour
